We are looking for an organised, experienced and enthusiastic Team Leader (Transport and Social Support Individual) within the Mandurah office.
Our goal is to provide safe, reliable and enjoyable Transport/ Social Support Individual (SSI) services in order for our valued clients to remain active and supported within the community.
Full time role: Within office hours - Monday to Friday.
Pay rate: $38.74 per hour + Salary packaging and Superannuation
In this role you will:
- Undertaking day to day oversight and administration the Transport and SSI programs consistent with government guidelines.
- Planning and scheduling the SSI and Community Transport.
- Ensuring that appropriate client records and data on services is maintained and that regular evaluations of client need and service delivery occur.
- Ensuring that client information is up to date and available for employees and/or volunteers.
- Coordinating client appointment and community transport requests.
- Developing service delivery plans in collaboration with clients.
- Developing, implementing, monitoring and scheduling vehicle maintenance.
- Upholding wellness and Client focused service delivery principles in all activities/tasks.
- Work with the Volunteer Team Leader to provide support the wider team.
What you will need:
- Working knowledge of the My Aged Care portal and process.
- Knowledge of relevant community resources.
- Ability to use computer systems and software (Nightingale exposure is an advantage)
- Excellent written and verbal communication skills.
- Ability to work with individuals from a range of social, financial and cultural backgrounds.
- Excellent organisational skills.
- Ability to work in a busy environment.
- Appropriate Australian Working Rights.
- Current Western Australian Drivers Licence.
- Current national police clearance (not older than 6 months).
Desirable:
- Current First Aid and CPR Certificate.
- Certificate IV or higher in Community Services or equivalent.
- Experience working in a not for profit organisation
- Experience overseeing volunteers and staff
